If You Don’t...

References and Resources

File a claim if the employee is covered under Basic Groups Life, or Optional Group Life. Include the Claim Information Claim Form, and one certified copy of the death certificate. You will not receive the death benefit in a timely manner Call Human Resources

If death was due to an accident covered by the Group Accidental Death and Dismemberment Plan, file a claim. Provide Proof of Loss Claim Form, one certified copy of the death certificate and Authorization to Release Information forms signed by the beneficiary. Exercise any vested stock options within one year of the date of death, or the expiration date, whichever is earlier. You will forfeit any options that are not exercised. Your Stock Option Grant Agreement

File any outstanding medical or spending account claims. In the case of spending accounts, the IRS stipulates that any unused money remaining in the account(s) after March 31 of the following year will be forfeited. Call Human Resources
