If the company no longer employs you, you are considered a terminated employee. There are some important things to consider.

Check with Human Resources to see what policies and procedures need to be completed. You may miss out on important information. Call Human Resources
Consider your 401(k) Plan loan repayment options Call your 401(k) plan administrator to obtain loan payoff information. Your outstanding loan may be defaulted. The remaining principal is due and payable within 90 days of termination. Benefits guide

Review the COBRA notice for continuation of medical, dental, vision and/or spending accounts and send the continuation enrollment for back as indicated in the instructions. Your coverage will end on the last day of the month in which our termination occurs (or your last paycheck for spending accounts). Benefits guide

If you do wish to continue coverage under COBRA, you will need to complete and mail the Extension of Coverage form within 60 days after your termination and mail your payment by the first of each month. If you have vested stock options, you must initiate the exercise prior to the expiration date as defined in your grant agreement. You will forfeit any options that are not exercised. Call the Stock Option Administrator
Your stock option agreements
File all claims under medical, dental, vision and spending accounts incurred prior to your termination date, if you are a participant. Claims will be ineligible for reimbursement after March 31 of the next calendar year for spending accounts and December 31 for the next calendar year for medical and dental. Out-of-network vision claims must be files within 6 months of the date incurred. Benefits guide

Change your group life, accident and disability insurance policies to individual policies. You may not be covered after termination of your employment. Call Human Resources

If you will be changing your address, call Human Resources to update you records. Any required mailings (W-2) will be mailed to the address on file. Call Human Resources
