Review your benefits guide and other enrollment materials. You may not understand your options or responsibility. Benefits guide
Enrollment materials
Fill out your benefits enrollment worksheet. This includes:
Your name, address and other information
Emergency contacts information
Tax withholding
You will not have an opportunity to enroll in any medical, dental, vision or spending accounts until the next annual open enrollment period. Late enrollment for life insurance plans and Long Term Care Disability may occur at any time, but only after showing proof of good health. Call Human Resources
Benefits guide
Complete Beneficiary Designation Claims would be paid according to a default provision. Call Human Resources
Start a health care and/or dependent care FSA You will have to wait until the next annual open enrollment to create or designate pre-tax dollars to an FSA account. Call Human Resources
Complete direct deposit form for payroll. You will receive a check each pay period rather than direct deposit. Payroll department
Call to request rollover instructions if you would like to roll your prior 401(k) or Pension into the company 401(k) Plan Your prior balance will be treated as per your prior plan rules. Call the 401(k) plan administrator
Call Human Resources
Carefully review your payroll stub to ensure proper deductions have been taken. Pay particular attention when you make any benefit or tax changes. You will not notice if any mistakes were made when processing your elections. Pay Stub
