Spouse or Domestic Partner Loses Eligibility

If your spouse changes jobs, their benefits may change, which may affect your benefits choices. To protect your family, be sure to make the proper benefits adjustments.

Consider enrolling spouse or domestic partner in medical, dental and vision upon termination of benefits. You will not be able to enroll you spouse or domestic partner until the next annual open enrollment Call Human Resources

If domestic partner is to be added, complete Declaration of Domestic Partnership form. You will not be able to enroll your domestic partner. Call Human Resources
Consider enrolling spouse or domestic partner in group life insurance upon termination of benefits. You may be required to provide statement of health. Call Human Resources

If spouse or domestic partner has eligible dependents, consider enrolling in medical, dental, vision and dependent group life insurance. If you do not complete the changes within 30 days of the change in family status, you will not be allowed to make changes until the next open enrollment period. Call Human Resources

Because your spouse’s change in benefits may affect your expenses, consider adjusting your contributions to your health care FSA. Your monthly contribution will continue to reflect your spouse’s designations. Call Human Resources
Review your beneficiary designation for Basic Group Life, Optional Life, AD&D, Long Term Care and 401(k). You may change designation at any time by completing the forms. Your existing beneficiary elections will remain in place. Call Human Resources
