Terminate Location Confirmation

You have elected to terminate a location. You are required to confirm that you want to terminate the location.

1.	Enter the Termination Date. Please note that the termination date must be on or after the most recent effective date. For example, if you created or modified this location on 03/01/2003, the termination date must be 03/01/2003 or later.
2.	Click Confirm to terminate the location. To cancel the request, click Cancel or the Back button on your browser.
