Edit Enrollment Information

To edit enrollment information:

1. Make necessary changes to the enrollment information.
2. Click Continue.


To add a dependent: (if applicable)

1. Click Add, located at the bottom of the page.

To access the provider directory web site: (if applicable)

1. Click the provider directory link, located at the bottom of the page.

To download and view a form: (if applicable)

1. Click the name of the form you wish to download.
2. With most browsers, a Save As window will appear, select the destination folder and click Save.
3. Once the download is completed, you can open the file by double-clicking on the name of the file.
