Plan Cost
The plan cost is the amount of money required to be paid to an insurance provider for services rendered. The cost is determined by the type of coverage.
| 1. | Select the applicable coverage levels for this plan. |
| 2. | Enter the coverage name. For most fields, you can either type a value, or select a pre-defined value from the drop down list below the tier field. |
| 3. | Enter the monthly cost (employee and employer contributions). |
| 4. | Click Continue. |
