To visit carrier's web site (if available):
| 1. | Click the URL (web address) of the carrier. Located below the carrier name. |
To download and view a form: (if applicable)
| 1. | Click on the name of the form you wish to download. |
| 2. | With most browsers, a Save As window will appear, select the destination folder and click Save. |
| 3. | Once the download is completed, you can open the file by double-clicking on the name of the file. |
